It is time to update your student’s required documentation for continued enrollment in the Licking Heights Local Schools District and to make new families in the district aware of the need to enroll your student prior to the start of the 2024-2025 school year.
Required Annual Forms
The window for completing these forms for the 2024-2025 school year will open on Tuesday, June 18, 2024. Emergency Medical Authorization (EMA) forms contain critical information to ensure the safety of your child while on school grounds, during school field trips, and in case of a medical emergency. There are also four additional forms that have to be completed or updated prior to your student returning to or starting school. If you have filled out these forms prior to this letter, you will need to fill them out again for the 2024-2025 school year, as the forms are required to be updated annually. If you are a newly enrolled student for the 2024-2025 school year these have already been completed.
Documents Required: There are a total of 5 forms to be submitted/updated and filled out entirely, even if there are no changes from the previous years. Forms A-E are all required to be completed annually as of this school year:

It is imperative that medical information is on file with Licking Heights Local Schools for the safety of your child. We are asking that the EMA forms be completed in PowerSchool by August 9, 2024. Failure to complete your child’s EMA will result in the following:
—No access to schedule or homeroom teachers
—No access to grades or attendance
—No performance in any extracurricular activities, including field trips
—Exclusion from attending school

Enrollment & EMA Support
If you have any questions regarding login information,
translators will be available throughout the summer on specific days and times at the district office located at 6539 Summit Road SW, Pataskala, Ohio 43062. These dates and times are listed in
this calendar. The earlier you complete your forms the better, so please don’t wait.
The district will also hold four all-day sessions in July and August to help parents complete their EMA forms and other required forms.
