Immunization Clinics

IMMUNIZATION CLINICS

According to the Ohio Revised Code 3313.67 and 3313.671, the parent or legal guardian is required to submit written documentation that their child has had all required immunizations to remain in school. Parents/guardians have 14 days to present evidence of compliance with the Ohio Revised Code.
